Despite recent improvements, current analysis confirms that millions of mothers and children are still dying every year because of severe anaemia due to insufficient blood supply. The lack of blood to treat severe perinatal haemorrhage contributes to up to 72% of maternal deaths (2,3). Similarly, delayed transfusion has been associated with increased infant mortality in cases of paediatric malaria-associated anaemia (4,5). Indeed, safe and reliable blood and blood products remain unavailable to many people living in the world’s poorest countries, particularly in SSA. While the need for blood is universal, there is a significant imbalance between developing and industrialized countries accessing safe blood.

Global Hunger Index (GHI) brings us face to face with a grim reality. The toxic cocktail of conflict, climate change, and the COVID-19 pandemic had already left millions exposed to food price shocks and vulnerable to further crises. Now the conflict in Ukraine—with its knock-on effects on global supplies of and prices for food, fertilizer, and fuel—is turning a crisis into a catastrophe. But the speed and severity of the global food crisis reflects the fact that millions of people were already living on the precarious edge of hunger—a legacy of past failures to build more just, sustainable, and resilient food systems. This year’s report therefore focuses on food systems transformation and local governance.
According to the 2022 GHI, Hunger is at alarming levels in 5 countries—Central African Republic, Chad, Democratic Republic of the Congo, Madagascar, and Yemen— and is provisionally considered *alarming *in 4 additional countries— Burundi, Somalia, South Sudan, and Syria. In a further 35 countries, hunger is considered serious, based on 2022 GHI scores and provisional designations.

As part of the recommendations, the Secretariat was requested to “explore the feasibility of a replenishment mechanism to broaden further the financing base, in consultation with Member States and taking into consideration the Framework of Engagement with Non-State Actors; and to present a report that includes relevant options for Member States to consider, to the Seventy-sixth World Health Assembly, through the 152nd session of the Executive Board and the thirty-seventh meeting of the Programme, Budget and Administration Committee in January 2023” (paragraph 39(f) of Appendix 2 of the Working Group’s report). In response to this request, the Secretariat reviewed the feasibility of a WHO replenishment mechanism in line with the principles set out by the Working Group on Sustainable Financing. It consulted with Member States through the work of the Agile Member States Task Group on strengthening WHO’s budgetary, programmatic and financing governance and benchmarked a set of replenishment mechanisms within and beyond the global health arena. This report outlines the Secretariat’s review and proposals on key elements of a potential WHO replenishment mechanism.

It does not make claims about the legitimacy of government decisions to restrict access – indeed, in many instances, there appeared to be a clear objective of limiting the spread of COVID-19 – but instead focuses on how access limitations have affected the delivery of aid.
