Health LAUNCH is an open innov...ation process that seeks to discover new solutions to intractable development challenges facing the Indo-Pacific region. The programme will focus on malnutrition caused by poor food intakes and seeks to (i) reduce the amount of permanent stunting and wasting and under 5 deaths caused by under nutrition in the Indo-Pacific and (ii) reduce the incidence of non-communicable disease caused by malnutrition. Currently up to 50 per cent of all child hood deaths in the region are caused by under nutrition and up to 1 in 2 children are physically or mentally stunted as a result of chronic under nutrition in the first thousand days of life. 98 per cent of all Tongans are at medium to high risk of premature death as a result of non-communicable disease. This initiative is a co-investment between Australia's Department of Foreign Affairs and Trade and USAID valued at $2.3 million over 3 years, starting 2015-16.

The UK Department of Health an...d Social Care (DHSC) funds outstanding global health research through the National Institute for Health Research (NIHR). The NIHR provided funding to the European and Developing Countries Clinical Trials Partnership (EDCTP) which aims to support collaborative research that accelerates the clinical development of new or improved interventions to prevent or treat poverty related as well as emerging and re-emerging infectious diseases affecting sub-Saharan Africa. NIHR has supported research as part of the 2018 Workplan focussed on: 'Advances in product development for effective prevention, treatment and management of co-infections and co-morbidities'; 'Diagnostic tools for poverty-related diseases Challenge'; and Vaccines for diarrhoeal diseases or lower respiratory tract infections . This activity is a component of European and Developing Countries Clinical Trials Partnership (EDCTP) phase II programme (GB-GOV-10-EDCTP2).

Le district de Nilphamari, dan...s le nord du Bangladesh, est l'une des régions les plus pauvres du pays. La plupart des habitants sont des travailleurs de terrain sans terre ou des travailleurs journaliers. Si la lèpre devient visible parmi eux, ils courent le risque d'être rejetés par leur environnement et de perdre leur emploi. Ce sont précisément ces craintes qui empêchent de nombreuses personnes de se faire soigner. Dans l'hôpital du district de Nilphamari, environ 200 opérations sont effectuées chaque année. En 2017, environ 16 418 personnes au total recevront des soins médicaux. L'objectif du projet est de fournir des soins de santé aux lépreux et aux personnes handicapées dans le besoin, en coopération avec l'hôpital local, et de promouvoir leur indépendance. Cela se fait par le traitement spécialisé des complications de la lèpre, les soins médicaux pour toute la population (pour les patients indigents) ainsi que par la formation médicale du personnel des services locaux de lutte contre la lèpre.

Despite remarkable progress ac...hieved in decreasing HIV transmission and AIDS-related deaths in the last decade due to development of over 30 antiretroviral drugs, HIV continues to spread in a virtually uncontrolled manner. An effective HIV vaccine remains one of the priorities of HIV/AIDS research and will always be the best solution and likely key to any strategy for halting the AIDS epidemic. The biggest challenge in developing such a vaccine is the enormous HIV variability, which dwarfs that of any other virus except hepatitis C virus. However, all parts of HIV cannot easily change, to remain alive, HIV has to keep some smaller regions of its proteins more or less constant to maintain function. Our vaccine strategy takes advantage of this and focuses the body defenses on these conserved parts of HIV, its Achilles heel. Because conserved regions are common to most of the global HIV variants, the vaccine, if successful, could be used in Africa, Asia, Europe, America and Australia: it would be universal. The 1st generation conserved-region vaccine was very safe and induced strong immune responses in adult volunteers in the UK and Africa. In 2014, the pharmaceutical company GSK acquired a small biotech company, which developed one of the benign viruses we used for vaccine delivery, and unilaterally decided that we could no longer use the jointly owned vaccine in high risk people. Thus we needed to switch to an alternative vaccine virus and this provided an opportunity to also substantially improve the 1st generation vaccine. These modifications are based on our experience from human vaccine testing and other developments in the HIV vaccine field over the last 10 years since its original design. Thus, the 2nd generation vaccine was born, using a new vaccine virus (ChAdOx1) owned by Oxford University and combining conserved regions with computer-designed proteins (mosaics), which significantly increase the vaccine match to global HIV variants. (Note that even the highly conserved regions are still somewhat variable.) Vaccines should match circulating HIVs as much as possible to stop them efficiently. The 2nd generation vaccines have been constructed, shown to induce strong immune responses in animal models and are now bound for testing in human volunteers. The requested funds will allow to us see how well the 2nd generation vaccine works in trying to get rid of HIV from already infected individuals and for larger scale testing in HIV-1 negative healthy volunteers in Africa. The latter study will aim first evidence that the vaccine can stop HIV from infecting healthy people. The funding will also allow clinical testing of the vaccine enabling further improvements including some new aimed at keeping ahead of HIV.

We are the NIHR Group on the M...anagement and Prevention of HIV-infection, Diabetes and Hypertension. In our NIHR Group award we had an ambitious objective to bring together vertically delivered HIV services (generally well-resourced and protected) and diabetes and hypertension care (currently less structured and poorly resourced), under one roof in Tanzania and Uganda. This approach was innovative, unique and risky. With support and in partnership with policy makers and disease control programme managers, we have successfully established and are currently evaluating this integrated care clinic model. However, the prevalence of chronic conditions is so high that clinic-based care alone cannot meet demand. Our NIHR Group is being urged by health services to evaluate decentralising integrated care to the community-level.Our primary research question for the proposed research is:How can we design community-based integrated management of HIV-infection, diabetes and hypertension, and what is its effectiveness in comparison to clinic-based integrated management of these conditions in terms of patient outcomes?Moreover, we want to know how an effective model of community-based integrated management can be scaled up and sustained across different country contexts in Africa.The proposed research will likely be more challenging than clinic-based integration, in part, because examples of community management, particularly of an integrated nature, are rare. The agenda has been defined after almost 2 years of discussions with African health policy makers in Tanzania and Uganda, and with input from patient and community representatives. The outputs from this activity can be applied/adapted for other DAC-list country/countries and therefore would benefit a wider geographical region.

The bacterium, Streptococcus p...neumoniae (pneumococcus), is the major cause of pneumonia-related deaths worldwide. The pneumococcus resides in the nose and at the back of the throat, where it does not normally cause disease, but when conditions change it causes life-threatening illness, such as pneumonia. The pneumococcus is transmitted from person to person via the respiratory route. In Europe and North America, immunisation of children with the pneumococcal conjugate vaccine (PCV13) has greatly reduced the burden of pneumococcal disease and prevalence of the pneumococcus in the community because children no longer infect adults. Similar impact has not been observed in low income countries, such as Malawi, because children continue to carry pneumococci even after vaccination. Instead, the prevalence of the pneumococcus among HIV-infected adults on antiretroviral therapy (ART) has remained high despite the introduction of PCV13 as routine childhood vaccine in Malawi in 2011. Persistence of pneumococcus among HIV-infected adults on ART threatens to reverse the potential public health benefits of PCV13 that have been reported from high income countries. HIV-infected adults on ART serve as a haven for pneumococcus and may promote transmission of the bacterium in the community. However, the factors that promote persistence of pneumococcus among HIV-infected adults on ART are still unknown. I propose to recruit 100 Malawian adults, of which half will be HIV-uninfected and the other half will be ART-naïve HIV-infected. I will collect samples from the nose from all study participants and follow them up for 12months. I will quantify the number of times an individual is found with the pneumococcus in their nose, and compare this between the study groups. I will also investigate differences in immune parameters in the nose between the study groups. This comparison will also focus on relating the changes in the immune parameters in the nose with the number of times an individual is found with the pneumococcus in their nose. This will generate information on some of the factors that make HIV-infected infected adults on ART more susceptible to the pneumococcus. Furthermore, I will grow cells obtained from the nose of HIV-infected and uninfected adults in laboratory to generate a deeper understanding of how they interact with pneumococcus. I anticipate that this research study will will improve current understanding of why the pneumococcus is persistently found among HIV-infected adults on ART. This work will allow choices in optimal therapy against HIV and optimal vaccination strategies against pneumococcus, that could significantly impact pneumococcal transmission and disease in high transmission settings.
