The Canadian National Microbio...logy Laboratory's (NML) National HIV & Retrovirology Laboratory (NHRL)'s program for Quality Assessment and Standardization of Immunology for HIV/AIDS treatment (QASI), is a quality assessment program for T cell subset enumeration (HIV diagnosis). The goal of QASI is to support patient HIV testing programs in countries where such supportive infrastructures are unavailable. The need for continued support to these sites is essential for patient care and linked closely to the global public health efforts to reach the 90-90-90 Treatment Targets. QASI evaluates the testing performance of participating laboratories 3 times per year through the provision of specimen proficiency panels. Participants test the samples and submit the results to the NHRL for evaluation. QASI provides the results together with detailed performance history and, when required, supports extensive remedial action to assist the participant sites with improvements to patient testing capacity. Where available, the NHRL strives to engage a single coordinating laboratory within participating country (frequently a national HIV laboratory) to serve as the central hub in a national testing program. The goal is to build a country's capacity to manage its own testing program for POC and non-POC diagnostics. NHRL/QASI has grown to more than 1300 participating sites enrolled in resource-limited countries. QASI works through 23 coordinating centers and provides service to 43 countries globally. The largest participant base is in Africa, with 15 coordinating centers and 21 participating countries, as well as 2 additional countries requesting enrolment. The increase in rapid HIV testing available in regions combined with the roll out and expansion of POC testing have both led to the significant increase in requests for participation in the QASI program. There is a critical need for external quality assessment programs to ensure that POC sites are accurately supporting patient treatment.POC testing allows for HIV testing to be carried out in the field with results provided in real time. With QASI's experience in assisting in resource-limited settings, POC sites can provide accurate and timely guidance for patient management for effectiveness of treatment in achieving viral suppression. In 2013 NHRL/QASI began development of a new arm of the QASI program focused on viral load proficiency testing. This technology is extremely important and reliable for effective patient care and for evaluating durable viral suppression through treatment. In addition, viral load and specifically POC viral load testing will be critical for early infant diagnosis.

Increased HIV drug resistance ...(DR) has been observed in Latin America and Caribbean (LAC) region. In the last decade, the Pan-America Health Organization (PAHO) has been supporting countries in strengthening their capacity to establish HIV DR surveillance strategies, nevertheless, due to technical and funding limitations, effective HIV DR surveillance remains inaccessible to some areas in LAC. The Canadian National Microbiology Laboratory (NML) researchers have developed a next-generation-sequencing (NGS)-based HIV DR testing technology which offers significantly enhanced detection sensitivity, data throughout and cost reduction in comparison to conventional approaches. Through coordination from PAHO, this project aims to implement this innovative technology into HIV DR surveillance practice in LAC. This project brings an innovative HIV drug resistance (DR) testing technology, developed by NML researchers, into public health practice. It fulfils the Public Health Agency of Canada (PHAC) mandate to serve as a central point for sharing Canada's expertise with the rest of the world, and represents a new PHAC contribution to enhance HIV/AIDS public health beyond our borders, especially in resource-limited countries. Thus far, 11 national/regional HIV reference laboratories from 8 LAC countries (Mexico, Brazil, Argentina, Chile, Colombia, Puerto Rico, Peru, Martinique) have been enrolled and a series of technology transfer activities have been conducted. Participating laboratories are all national/regional HIV drug resistance (DR) reference labs performing HIV genotyping for surveillance, research and clinical monitoring purpose in their respective countries; they include five WHO accredited laboratories from the Global ResNet HIV Genotyping Network. In accomplishing this project, these participating labs will acquire full capability of performing NGS-based HIV DR testing using Illumina MiSeq platform independently for more affordable HIV DR surveillance and clinical monitoring.

NHRL/QASI has grown to more than 1300 participating sites enrolled in resource-limited countries. QASI works through 23 coordinating centers and provides service to 43 countries globally. The largest participant base is in Africa, with 15 coordinating centers and 21 participating countries, as well as 2 additional countries requesting enrolment. The increase in rapid HIV testing available in regions combined with the roll out and expansion of POC testing have both led to the significant increase in requests for participation in the QASI program. There is a critical need for external quality assessment programs to ensure that POC sites are accurately supporting patient treatment. POC testing allows for HIV testing to be carried out in the field with results provided in real time. With QASI's experience in assisting in resource-limited settings, POC sites can provide accurate and timely guidance for patient management for effectiveness of treatment in achieving viral suppression. In 2013 NHRL/QASI began development of a new arm of the QASI program focused on viral load proficiency testing. This technology is extremely important and reliable for effective patient care and for evaluating durable viral suppression through treatment. In addition, viral load and specifically POC viral load testing will be critical for early infant diagnosis.
