Community led-monitoring is based on the principle that «Nothing that is done for us should
be done without us”. The combination of this principle with evidence shows that community-led
monitoring is an important driver of improved service delivery and health outcomes that needs to
be re-empha...sized. Thus, the community must participate at all stages of the fight against malaria.
This guide will be useful to CSOs working in the field of malaria in the conduct of community-led
monitoring of activities efficiently and allow these CSOs to know their role and responsibilities in this
exercise at each key stage. This guide will also provide CSOs and communities affected by malaria
with templates of monitoring tools adapted to key malaria programs.

Le paludisme reste un enjeu majeur de santé publique dans de nombreux pays à faible et moyen revenu. En 2018, l’OMS estimait à environ 228 millions le nombre de cas de paludisme, responsable de 405 000 décès principalement en Afrique subsaharienne. L’utilisation intensive des outils antipal...udiques disponibles a permis de réduire significativement la transmission dans de nombreuses régions endémiques au cours de la dernière décennie. Cependant, l’éradication du paludisme dans les zones à forte transmission reste une perspective lointaine.
Ce MOOC offre aux participants un aperçu complet des diverses disciplines et questions clés de la recherche sur le paludisme. Il aborde notamment la biologie cellulaire et moléculaire des parasites, la recherche clinique sur le terrain et en milieu hospitalier, les technologies génomiques avancées, la recherche translationnelle sur les nouveaux outils de lutte contre les parasites et vecteurs (médicaments, vaccins), l’épidémiologie du paludisme et la recherche clinique en santé publique. Le cours est accessible à toute personne intéressée par le paludisme, à condition de posséder une formation initiale en sciences de la vie.

Vector control, alongside case management, remains the most effective approach to controlling and eliminating malaria. La gravité de la récente situation épidémiologique en Amérique latine, marquee par la co-circulation des virus de la dengue, du chikungunya et du Zika, l’émergence d’épizootie de fièvre jaune et l’apparition de cas de microcéphalie et d’autres maladies associées (p. ex. le syndrome... de Guillain-Barré) et l’émergence d’épizooties de fièvre jaune ont amené l’Organisation mondiale de la Santé à déclarer une situation d’urgence dans les Amériques en 2016.1 En l’absence d’un traitement spécifique et de vaccins contre la dengue, le chikungunya et le virus Zika, et compte tenu des limitations des stratégies de lutte antivectorielle, l’OMS a préconisé une meilleure utilisation des alternatives disponibles pour améliorer la lutte contre le moustique vecteur Aedes aegypti, ainsi que des activités complémentaires.

Les manuels de formation TACT et la brochure destinée aux patients ont été élaborés dans le cadre de l'initiative du consortium ACT visant à améliorer la prise en charge du paludisme en encourageant l'utilisation correcte des tests de diagnostic rapide (TDR). L'initiative TACT (Targeting ACTs...) vise à garantir l'utilisation appropriée des combinaisons thérapeutiques à base d'artémisinine (ACT), en ne traitant que les cas de paludisme confirmés et en orientant les patients vers d'autres traitements pour les maladies fébriles non liées au paludisme. Les manuels proposent une formation interactive sur le lieu de travail pour aider les personnels de santé à passer d'un traitement présomptif à un diagnostic fondé sur des tests, conformément à la politique de l'OMS. La brochure d'accompagnement destinée aux patients, disponible en anglais et en swahili, s'appuie sur des récits illustrés pour expliquer l'utilité et le fonctionnement des TDR, répondre aux inquiétudes les plus courantes et encourager la confiance dans les résultats des tests. Ces documents, dont la clarté et la compréhension ont été testées au préalable, permettent d'opérer des changements durables dans la pratique clinique et la communication avec les patients au sein des établissements de soins primaires tanzaniens.

A iniciativa TACT ("Targeting ACTs") visa garantir a util...ização adequada das terapias combinadas à base de artemisinina (ACT), tratando apenas os casos de malária confirmados e orientando os cuidados alternativos para outras doenças febris. Os manuais oferecem formação interativa, baseada no local de trabalho, para auxiliar os profissionais de saúde na transição do tratamento presuntivo para um diagnóstico baseado em testes, em conformidade com a política da OMS. O folheto para o paciente, disponível em inglês e suáili, utiliza histórias ilustradas para explicar a importância e o objetivo dos TDR, abordar preocupações comuns e incentivar a confiança nos resultados dos testes. Após terem sido pré-testados quanto à sua clareza e compreensão, estes materiais apoiam mudanças sustentáveis na prática clínica e na comunicação com os doentes no âmbito dos cuidados de saúde primários na Tanzânia.
