Os sistemas de saúde estão enfrentando um rápido aumento na demanda gerada pelo surto da COVID-19. Quando os sistemas de saúde ficam sobrecarregados, há um aumento drástico na mortalidade direta causada por um surto e também na mortalidade indireta por doenças imunopreveníveis e por aquelas... doenças que possuem tratamento. Análises do surto de ebola em 2014-2015 sugerem que o aumento no número de óbitos causados por sarampo, malária, HIV/AIDS e tuberculose atribuíveis a falhas no sistema de saúde ultrapassou o número de óbitos causados pelo ebola. A capacidade de um sistema de manter a prestação de serviços essenciais de saúde dependerá de sua capacidade inicial e da carga da doença e do contexto de transmissão do vírus COVID-19 (classificado como nenhum caso, transmissão esporádica, em clusters ou comunitária). Manter a confiança da população na capacidade do sistema de saúde de atender, com segurança, as necessidades essenciais e de controlar o risco de infecção nas unidades de saúde é fundamental para garantir que as pessoas continuem a buscar atendimento quando necessário e que sigam as orientações de saúde pública.

Background
Asthma education, a key component of long-term asthma management, is challenging in resource-limited settings with shortages of clinical staff. Task-shifting educational roles to lay (non-clinical) staff is a potential solution. We conducted a randomised controlled trial of an enhanced a...sthma care intervention for children in Malawi, which included reallocation of asthma education tasks to lay-educators. In this qualitative sub-study, we explored the experiences of asthmatic children, their families and lay-educators, to assess the acceptability, facilitators and barriers, and perceived value of the task-shifting asthma education intervention.
Methods
We conducted six focus group discussions, including 15 children and 28 carers, and individual interviews with four lay-educators and a senior nurse. Translated transcripts were coded independently by three researchers and key themes identified.
Results
Prior to the intervention, participants reported challenges in asthma care including the busy and sometimes hostile clinical environment, lack of access to information and the erratic supply of medication. The education sessions were well received: participants reported greater understanding of asthma and their treatment and confidence to manage symptoms. The lay-educators appreciated pre-intervention training, written guidelines, and access to clinical support. Low education levels among carers presented challenges, requiring an open, non-critical and individualised approach.
Discussion
Asthma education can be successfully delivered by lay-educators with adequate training, supervision and support, with benefits to the patients, their families and the community. Wider implementation could help address human resource shortages and support progress towards Universal Health Coverage.

Dengue creates a staggering epidemiological and economic burden for endemic countries. Without a specific therapy and with a commercial vaccine that presents some problems relative to its full effectiveness, initiatives to improve vector... control strategies, early disease diagnostics and the development of vaccines and antiviral drugs are priorities. In this study, we present the probable origins of dengue in America and the trajectories of its spread. Overall, dengue diagnostics are costly, making the monitoring of dengue epidemiology more difficult and affecting physicians’ therapeutic decisions regarding dengue patients, especially in developing countries. This review also highlights some recent and important findings regarding dengue in Brazil and the Americas. We also summarize the existing DENV polymerase chain reaction (PCR) diagnostic tests to provide an improved reference since these tests are useful and accurate at discriminating DENV from other flaviviruses that co-circulate in the Americas. Additionally, these DENV PCR assays ensure virus serotyping, enabling epidemiologic monitoring.
