Les manuels de formation TACT et la brochure destinée aux patients ont été élaborés dans le cadre de l'initiative du consortium ACT visant à améliorer la prise en charge du paludisme en encourageant l'utilisation correcte des tests de diagnostic rapide (TDR). L'initiative TACT (Targeting ACTs...) vise à garantir l'utilisation appropriée des combinaisons thérapeutiques à base d'artémisinine (ACT), en ne traitant que les cas de paludisme confirmés et en orientant les patients vers d'autres traitements pour les maladies fébriles non liées au paludisme. Les manuels proposent une formation interactive sur le lieu de travail pour aider les personnels de santé à passer d'un traitement présomptif à un diagnostic fondé sur des tests, conformément à la politique de l'OMS. Le mémoire porte sur la conception et la mise en œuvre d’une campagne de communication pour promouvoir la chimio-prévention du paludisme saisonnier (CPS) chez les enfants de 3 à 59 mois au Cameroun, une stratégie recommandée par l’OMS dans les zones de forte transmission saisonnière. L’...auteure part du constat que, malgré l’introduction de la CPS dans certaines régions camerounaises, sa mise en œuvre reste confrontée à une faible appropriation communautaire, à des résistances sociales et à un manque de communication adaptée.
L’objectif du travail est de concevoir une stratégie de communication sociale et comportementale visant à renforcer l’adhésion des communautés à cette intervention. Le mémoire s’appuie sur une analyse situationnelle approfondie, intégrant des données épidémiologiques, socioculturelles et communicationnelles. Il propose ensuite une campagne ciblée, articulée autour de messages clairs, de canaux adaptés (radios communautaires, agents de santé, leaders locaux) et de supports visuels compréhensibles.
La campagne vise à sensibiliser les parents, en particulier les mères, sur les avantages de la CPS, à corriger les fausses croyances, à encourager l’observance du traitement, et à renforcer la confiance envers les agents communautaires. In November 2022, WHO launched a strategy to address antimalarial drug resistance in Africa, emphasizing the need for stronger surveillance. While regional monitoring networks have been active in other regions, most in Africa have not convened since 2017–2018. To renew collaboration, WHO organized... a meeting bringing together countries from the African and Eastern Mediterranean regions. Participants shared recent data on antimalarial efficacy and resistance. While artemisinin-based combination therapies remain highly effective, some sites in Uganda and the United Republic of Tanzania reported lower-than-expected efficacy of artemether–lumefantrine. Delayed parasite clearance linked to Plasmodium falciparum Kelch-13 mutations was noted in Eritrea, Rwanda, Uganda and the United Republic of Tanzania. Discussions highlighted challenges in therapeutic efficacy studies, molecular marker surveillance and the need for improved genotyping to distinguish relapses from new infections.

Pregnant travelers face numerous risks, notably increased susceptibility to or severity of multiple infections, including malaria. Because pregnant women residing in areas non-endemic for malaria are unlikely to have protective immunity, travel to endemic areas poses risk of severe illness and pregn...ancy complications, such as low birthweight and fetal loss. If travel to malaria-endemic areas cannot be avoided, preventive measures are critical. However, malaria chemoprophylaxis in pregnancy can be challenging, since commonly used regimens have varying levels of safety data and national guidelines differ. Furthermore, although chloroquine and mefloquine have wide acceptance for use in pregnancy, regional malaria resistance and non-pregnancy contraindications limit their use. Mosquito repellents, including N,N-diethyl-m-toluamide (DEET) and permethrin treatment of clothing, are considered safe in pregnancy and important to prevent malaria as well as other arthropod-borne infections such as Zika virus infection. Pregnant travelers at risk for malaria exposure should be advised to seek medical attention immediately if any symptoms of illness, particularly fever, develop.

Antimalarial chemotherapy is crucial for reducing morbidity, mortality, and drug resistance, and is the cornerstone of malaria control. Existing antimalarial drugs act at different stages of the parasite’s life cycle. These drugs range from classic agents such as chloroquine and quinine to newer a...rtemisinin derivatives. They include tissue schizonticides, blood schizonticides, gametocytocides, and sporontocides. Artemisinin and its derivatives are the most effective and fastest-acting treatment against drug-resistant Plasmodium falciparum, achieving rapid parasite clearance and reducing transmission potential. Other key drugs include mefloquine, halofantrine, proguanil, sulfadoxine–pyrimethamine, atovaquone–proguanil, tetracyclines, clindamycin and azithromycin. Each of these drugs has a specific mechanism of action, pharmacokinetics, efficacy, safety profile and contraindications. Rational drug combinations and adherence to national treatment guidelines are essential for managing resistance, ensuring safety in vulnerable populations such as children and pregnant women, and optimising therapeutic outcomes in cases of both uncomplicated and severe malaria.

Combination therapy is a cornerstone of modern malaria treatment, particularly in the context of widespread multidrug resistance. Using two or more antimalarial drugs with different mechanisms simultaneously enhances efficacy, shortens treatment duration, improves compliance and delays the developme...nt of resistance. Artemisinin-based combination therapies (ACTs), such as artemether–lumefantrine, artesunate–amodiaquine and artesunate–sulfadoxine/pyrimethamine, are highly effective in rapidly clearing parasites and reducing gametocyte carriage. They are also generally well tolerated. Non-artemisinin combinations, quinine-based regimens and novel combinations (e.g. piperaquine–dihydroartemisinin) offer alternative therapeutic options, although clinical experience with these remains limited. Although ACTs are the preferred first-line treatment, factors such as cost, local drug resistance patterns, safety during pregnancy and paediatric use must inform implementation and policy decisions.

The African Palliative Care Association is pleased to publish the first edition of Palliative Care Standards for Africa. The development of these standards was achieved through wide consultation with service beneficiaries and providers, and they have been developed to suit different levels of ... service delivery, from primary to tertiary. These standards are underpinned by the World Health Organization’s definition of palliative care, and recognise that scaling up palliative care requires a public health approach with four pillars: policy, education, drug availability and implementation. In addition, the increasing need to establish specific indicators of quality and effectiveness for palliative care has been a big driving force behind these comprehensive standards. It is APCA’s wish that they will provide a framework for the development of evaluation
and performance indicators that can facilitate programme improvement and development. The standards are designed to allow the development or improvement of palliative care across the different services levels, within the organisational capacity of various service providers. They describe a relationship between primary, intermediary and tertiary level service providers, with expectations for all providers articulated through detailed criteria for each standard. It is therefore expected that these standards will influence the planning and delivery of palliative care services at all levels of health care service delivery.

The Lao People’s Democratic Republic experienced substantial economic growth in the 30 years prior to the coronavirus disease (COVID-19) pandemic, contributing to reduced poverty and significant progress toward the SDGs. However, the COVID-19 pandemic brought this development to a halt. It was anticipated that the COVID-19 recovery and the tremendous population growth in recent years would provide opportunities for a shift toward more sustainable and inclusive development in the years ahead. In 2023, however, the contrary was the case. Rural residents, including many ethnic minorities, continued to face marginalization because of limited access to education, health care and economic opportunities.
Despite the challenges of COVID-19 and other disease outbreaks, the country has made significant improvements in health. Nonetheless, progress has been uneven and not everyone has benefited from these achievements. In the mountainous region, many people lack access to quality health care because of the unequal distribution of well-trained health-care workers. Preventable deaths due to poor-quality health care for children and newborns, infants and mothers remain a concern, as do communicable diseases such as sexually transmitted infections and tuberculosis. The increasing burden of noncommunicable diseases and the health impact of worsening climate change further heighten the need for strengthened and resilient health systems, which are at risk due to an underfunded health sector and weak economy.
This CCS aims to address remaining and future challenges as well as health needs while creating an impact that is sustainable. It identifies three strategic priorities and nine deliverables (Table 1) to support the attainment of the national vision of Health for all by all, as articulated in the 9th Health Sector Development Plan 2021–2025. It contributes to the country’s goals to achieve universal health coverage, graduate from least developed country status by 2026 and attain SDGs by 2030.

Mounting an effective international humanitarian response to a chemical, biological, radiological or nuclear (CBRN) event, especially if the response is undertaken on an ad hoc basis, would be extremely difficult and would pose many risks to the responders. The International Committee of the Red Cro...ss (ICRC) has created a competency-based capacity to respond to at least small-scale CBRN events, including a deployable capability to undertake operational activities. This involves informed assessments of CBRN risks, timely and competent decisions on how to respond, and effectively mobilizing appropriate resources to implement these decisions, through the creation of an emergency roster. In addition to the acquisition of technical expertise and material resources, the creation of such capacity requires the application of central processes, ensuring systematic management of CBRN response (including risk-based decision-making), standing operational procedures, and availability of and access to the necessary resources. Implementation of the ICRC's CBRN response framework as described in this article should be considered by any agency or other stakeholder preparing for international humanitarian assistance in CBRN events – especially if such events are related to armed conflict.
