This paper was commissioned by N´weti and Wemos as part
of the project “Equitable health financing for a strong health
system in Mozambique”. Its purpose is to contribute to the
debate of the Mozambican Ministry of Health’s draft Health
Sector Financing Strategy (HSFS) 2025 – 2034
Over the past decade, countries in the African region experienced slow progress in mobilizing resources for health while facing continued challenges. In their revised estimates published in 2017, Stenberg et al., developed two costs scenarios, termed progress and ambitious, aimed at strengthening co...mprehensive health service delivery to achieve SDG 3 and universal health coverage in low-income and middle-income countries (Stenberg et al., 2017). Out of the 47 countries in the WHO African region only eight, on average, met the recommended threshold of spending a minimum of US$ 249 per capita on health during the period from 2012 to 2020. In 2020, this achievement was observed in only five countries while the remaining countries spent less than US$ 249 per capita, with health expenditures ranging from US$ 16.4 to US$ 236.6, highlighting significant disparities across the region.

En tant que département technique de l'OMS chargé du paludisme, le Programme mondial de lutte contre le paludisme a un rôle important à jouer dans la conduite de la riposte mondiale contre cette maladie. Grâce à ses actions directes et à son réseau, il peut façonner l'écosystème du paludi...sme et avoir un impact au niveau des pays. Dans cette optique, le Programme mondial de lutte contre le paludisme a élaboré une stratégie opérationnelle décrivant ses priorités pour la période 2024-2030 et les 4 leviers stratégiques pour contrôler et éliminer le paludisme qui relèvent de manière décisive du mandat du Programme : normes et critères, nouveaux outils et innovation, informations stratégiques axées sur les résultats et leadership. La stratégie décrit la façon dont le Programme mondial de lutte contre le paludisme se transformera également en la mise en place d’une collaboration plus efficace avec d’autres programmes, les bureaux régionaux et pays ainsi que les partenaires, en s’appuyant sur les enseignements tirés du treizième programme général de travail de l'OMS (GPW13) et des priorités du GPW14.

All malaria-endemic countries in the Region of the Americas have taken on the challenge to eliminate the disease and to put in place measures to orient their health programs and strategies in that direction. This manual explains how to implement measures to achieve malaria elimination and prevent it...s reestablishment by increasing the intensity and quality of interventions, reorienting initiatives, reducing delays that favor transmission, and ensuring adequate monitoring to adjust interventions. In this regard, the first suggested step is to stratify the territory - so that interventions and population groups can be planned and prioritized - and then implement diagnosis, treatment, investigation and response (DTI-R) actions as a key strategy aimed at effectively eliminating the disease and preventing its reestablishment in all strata. In addition, the document seeks to operationalize the concept of "surveillance as an intervention" promoted by the World Health Organization through the Global Technical Strategy for Malaria 2016-2030 and highlights the importance of communities having access to early diagnosis and treatment. Finally, the manual also includes management and monitoring aspects that must be taken into account to eliminate the foci with transmission.
