Tuberculosis (TB) control in the African Region has evolved since the disease was declared a global emergency by the World Health Organization (WHO) in 1993. Member States have adopted and implemented successive global and regional strategies and resolutions, with demonstrable positive impacts on in...cidence, prevalence and mortality, albeit with variations across countries. By the end of 2015, the Region as a whole met the key Millennium Development Goal (MDG) target of halting and beginning to reverse TB incidence. However only 35 of the 47 Member States met the MDG target.

HAT diagnosis relies on laboratory techniques because clinical signs and symptoms are unspecific. Serodiagnostic tests exist only for Tbg and are based on the detection of specific antibodies, thus they are not confirmatory of infection. With the current low disease prevalence, the positive predicti...ve value of serological tests is particularly low. Field-applicable tools include the card agglutination test for trypanosomiasis (CATT) used mainly in active screening by specialized mobile teams, and the rapid diagnostic tests that are more suitable for individual testing at point-of-care. Confirmation of Tbg infection requires microscopic examination of body fluids necessitating specific training. The best performing methods are laborious and reach 85–95% diagnostic sensitivity when performed by skilled personnel.

In November 2022, WHO launched a strategy to address antimalarial drug resistance in Africa, emphasizing the need for stronger surveillance. While regional monitoring networks have been active in other regions, most in Africa have not convened since 2017–2018. To renew collaboration, WHO organized... a meeting bringing together countries from the African and Eastern Mediterranean regions. Participants shared recent data on antimalarial efficacy and resistance. While artemisinin-based combination therapies remain highly effective, some sites in Uganda and the United Republic of Tanzania reported lower-than-expected efficacy of artemether–lumefantrine. Delayed parasite clearance linked to Plasmodium falciparum Kelch-13 mutations was noted in Eritrea, Rwanda, Uganda and the United Republic of Tanzania. Discussions highlighted challenges in therapeutic efficacy studies, molecular marker surveillance and the need for improved genotyping to distinguish relapses from new infections.
